Position Overview

The Licensed Practical Nurse (LPN) position is responsible for delivering quality patient care in a home health setting. This role involves performing skilled nursing functions in accordance with the State Nurse Practice Act, established organizational policies, and accepted standards of practice. The LPN will work collaboratively within a multidisciplinary team, including Registered Nurses (RNs), Physical Therapists (PTs), Occupational Therapists (OTs), and Home Health Aides (HHAs).

Key Responsibilities

• Provide skilled nursing care to patients as delegated by the RN Case Manager and/or Clinical Team Manager.
• Perform 6 patient visits per day, ensuring high-quality care and compliance with all applicable laws and regulations.
• Maintain accurate and timely documentation of patient care activities.
• Participate in team meetings and communicate effectively within the care team.
• Adhere to established organizational and departmental policies and procedures.
• Complete other duties as assigned within the scope of practice.

Work Schedule

This position typically requires work Monday through Friday from 8:00 AM to 4:30 PM, with a weekend rotation of one weekend per month. Flexibility in scheduling may be required to meet patient needs.

Qualifications

• Must have experience in home health care.
• Current LPN license in accordance with state regulations.
• BLS certification is required.
• Strong communication and interpersonal skills.
• Ability to work effectively as part of a team.

Physical Requirements

The role may involve physical demands such as lifting, bending, and standing for extended periods. The candidate must be able to perform these essential functions safely.
