Job Summary
We are seeking an experienced Case Manager to assess, evaluate, and drive discharge plans while coordinating services to ensure patients receive the appropriate level of care. The ideal candidate will actively monitor treatment plans and patient progress, identifying any barriers to discharge and ensuring compliance with regulatory guidelines.
Key Responsibilities
- Assess and evaluate patients to develop effective discharge plans.
- Coordinate services that align with the patient's specific level of care.
- Initiate, evaluate, and re-evaluate the treatment plan on a daily basis.
- Monitor patient progress and any lack thereof on a daily basis.
- Track length of stay and identify barriers to discharge.
- Adhere to Medicare regulatory guidelines concerning patient choice, IMM notices, and MOONS.
Schedule
This role is scheduled Monday through Friday, with no weekend or call requirements. Block scheduling is available.
Qualifications
- A minimum of 3 years of relevant experience is required; first-time travelers will not be accepted.
- Candidates will undergo a clinical screening process to determine fit for the role.
Additional Information
Requests for time off (RTO) should be a maximum of 7 days and discussed during the interview. Please note that no RTO will be permitted during September.
Travelers may be expected to work major holidays if they fall within their assignment period. A traveler is required to work one major holiday if only one falls during their assignment, and two major holidays if two fall within that time. Major holidays include Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, and New Year's Eve/New Year's Day.
Full compliance with orientation requirements is necessary to ensure timely start dates.
