Job Description

We are seeking an experienced Inpatient Float Case Manager to join our dedicated care coordination team. The selected candidate will be responsible for managing the care of patients during their acute inpatient stay, collaborating with interdisciplinary teams to ensure optimal care outcomes.

Responsibilities

• Conduct comprehensive assessments of patients' needs and develop individualized care plans.
• Coordinate patient care with multidisciplinary teams to ensure seamless transitions.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families about care plans and treatment options.
• Maintain accurate and up-to-date documentation using electronic health records.

Qualifications

• Valid Registered Nurse (RN) license.
• Acute inpatient experience required; Level 1 experience preferred.
• Proficiency in Epic electronic health record system is required.
• Strong communication and interpersonal skills.
• Ability to work collaboratively in a fast-paced environment.

Shift Details

This position requires working 4 ten-hour shifts, primarily on weekends (Saturday/Sunday) with flexibility for the other two days.
