Job Summary:
The RN – Case Manager provides skilled nursing care and case management for assigned patients in their place of residence. This role involves admitting patients onto services, coordinating and integrating care to ensure continuity, and facilitating quality clinical outcomes. The RN will work collaboratively with patients, families, and an interdisciplinary team to address healthcare needs effectively.
Essential Duties and Responsibilities:
- Provide skilled nursing and coordinate care for patients and their families in a residential setting, authorized by physician guidelines.
- Observe, assess, evaluate, and plan interventions based on patients’ physical and psychosocial statuses, including changes in conditions or reactions to treatments or medications.
- Oversee care provided by Home Health Aides and CNAs, ensuring compliance with the plan of care.
- Maintain effective communication and facilitate case conferences with other health disciplines, community health, and social agencies to assure continuity of care.
- Deliver quality care and appropriate symptom management to promote optimal health and quality of life through teaching, counseling, and referrals.
- Maintain accurate and complete patient records in accordance with Agency standards and regulatory requirements.
- Participate in team scheduling coordination and on-call rotation to meet patient care needs by assessing situations and determining the necessity of home visits.
- Engage in quality improvement activities and attend in-service training as required.
- Perform other duties as assigned by a supervisor.
Minimum Qualifications:
- Current RN license in the State of Montana (BSN preferred).
- Valid driver’s license, appropriate auto insurance, and reliable transportation.
- A minimum of 2 years of recent acute care, home care, or hospice experience, including skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferred.
- Experience with HCHB is strongly preferred.
Schedule:
Typical schedule is Monday through Friday, 8:00 AM to 5:00 PM, with a rotation for weekend and on-call coverage.
