Job Summary

The Case Management Registered Nurse will be responsible for overseeing discharge planning and ensuring effective case management within an acute care environment. The role involves collaborating with multidisciplinary teams to facilitate patient care and support positive health outcomes.

Responsibilities

• Manage discharge planning processes for patients in an acute care setting.
• Conduct assessments and develop individualized care plans in collaboration with medical staff.
• Coordinate with healthcare providers to ensure continuity of care post-discharge.
• Provide education and support to patients and families regarding discharge plans and community resource options.
• Monitor patient progress and adjust care plans as necessary.

Qualifications

• Active Massachusetts RN license or conditional license.
• Minimum of 2 years of case management experience.
• At least 6 months of experience in discharge planning within an acute care hospital setting, preferably at a large organization or teaching hospital with 500 beds or more.
• Strong communication and organizational skills.
• Ability to work collaboratively in a fast-paced environment.

Work Schedule

This position is full-time, with a schedule of 40 hours per week. Operating hours may vary based on organizational needs, with potential for either 4 ten-hour shifts or 5 eight-hour shifts, typically during daytime hours from Monday to Friday.
