Position Overview

We are seeking a dedicated Case Manager to join our acute care team. The Case Manager will be responsible for assessing and coordinating care for patients across various inpatient units, providing support to ensure optimal patient outcomes.

Key Responsibilities

• Collaborate with healthcare professionals to develop and implement patient care plans.
• Conduct assessments of patient needs and establish goals for care.
• Facilitate communication among patients, families, and the healthcare team.
• Monitor and evaluate patient progress and adjust care plans as necessary.
• Provide education and resources to patients and families to support care transitions.
• Assist with discharge planning and follow-up care coordination.

Work Schedule

This position is full-time, with shifts scheduled from Monday to Friday, 8:30 AM to 5:00 PM. Weekend work is required once a month, with the option to substitute a weekday shift for a weekend day.

Qualifications

• Minimum of 2 years of acute hospital experience is required.
• Experience with electronic health records, specifically Epic, is mandatory.
• Positions are open to first-time travelers as well as local candidates.
• Strong communication skills and the ability to work collaboratively with a diverse team.
• Ability to float between various inpatient units as needed.
