Position Overview

This position requires a dedicated Registered Nurse (RN) specializing in Home Health Care. The ideal candidate will have strong case management skills and experience in using the EPIC electronic medical record system.

Schedule

• Monday to Friday schedule
• Rotating every fourth weekend
• 3 to 4 on-call shifts per month, with callbacks required between 5 PM and 8 PM

Role Responsibilities

• Case management for 20-25 patients, expecting 5-6 visits per day
• Utilize EPIC for documentation and patient records
• Conduct comprehensive assessments and develop care plans for patients
• Coordinate with healthcare team members to ensure quality patient care
• Provide education and support to patients and their families

Qualifications

• Experience: Minimum of 2 years of nursing experience required, preferably in home health
• Certifications: CPR certification (BLS) is required
• Skills: Proficiency in EPIC and knowledge of OASIS is mandatory
• Preferred Skills: Experience with IV therapy, tube feeds, VAC dressings, compression wraps, port access, ostomies, chest tubes, urinary catheters, and blood draws

The Registered Nurse will primarily work with adult patients with a variety of medical conditions, including:

• Medical-surgical
• Oncology
• Cardiology
• Neurology
• Respiratory
• Wound care
• Gastrointestinal
• Genitourinary
