The RN Care Coordinator (RNCC) is responsible for managing a caseload of patients, ensuring that care is delivered in a manner that supports optimal clinical and financial outcomes. This role requires specialized skills and knowledge tailored to the unique needs of the assigned patients. The RNCC coordinates care as prescribed by an interdisciplinary team, using a comprehensive approach to patient assessment and care management.

Key Responsibilities

• Manage a designated caseload of patients, overseeing their care from pre-admission through post-discharge.
• Utilize patient assessments, care guidelines, and protocols to evaluate responses to therapies.
• Collaborate with an interdisciplinary team to coordinate care and ensure alignment with clinical goals.
• Participate in the ongoing evaluation of practice patterns and systems to enhance quality, cost-efficiency, and patient satisfaction.
• Mobilize resources effectively to optimize the efficiency of care delivery.

Qualifications

• Current and valid RN license.
• Experience in case management or care coordination preferred.
• Ability to assess and address the individual needs of patients.
• Strong communication and interpersonal skills.
• Knowledge of payer regulations and protocols impacting patient care.
