Description

Case Manager needed for 3 Central with 2+ years of acute hospital case management experience.
Responsibilities include discharge planning, chart review, patient screening, care coordination, and barrier mitigation.
Must be collaborative, organized, able to multitask, and communicate effectively with patients, staff, and the care team.
Experience with Meditech and Midas preferred.
