Description

RN Case Manager – Days

Shift: Monday–Friday, 8:00 AM – 4:30 PM
Experience: Minimum 2 years of recent RN Case Management experience required
Required Setting Experience: LTAC or STACH experience required
Required Certs/Licensure: BLS through AHA and North Carolina or multi-state RN license
Required Skills: MCG, utilization review, discharge planning, acute care case management, care coordination, and strong IDT collaboration
Responsibilities: Coordinate patient care, support resource utilization, complete assessments, assist with discharge planning, promote continuity of care, and advocate for patient needs/preferences

United Staffing Solutions Job ID #1006442. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
