RN Case Manager

Requirements: BSN, active RN license
Experience: 2+ years acute care experience preferred; inpatient case management required
Preferred: CCMC certification
Role: Coordinates patient care throughout hospitalization, manages discharge planning and transitions of care, and performs utilization management and quality reviews
Skills: Inpatient case management, managed care, discharge planning, care coordination, and interdisciplinary collaboration
