Description

Role: RN Case Manager (Inpatient Rehab Case Management) | Openings: 1

Schedule: 8:00a–4:00p | 40 hrs/week | Weekends/Call: None

Unit: 40 beds | Ratio: 1:13 | Patient Population: General

EHR: Cerner, UDS

Required Licensure/Education: Current RN or SW licensure (BSW/MSW) or licensed healthcare professional (RT/PT/OT/SLP)

Preferred Certifications: Case Management/Rehab certs (e.g., CCM, ARN, ACM, ANCC RN-BC)

Experience: 2+ years inpatient case management/social work (acute/rehab preferred); IPR experience strongly preferred

Must-Have Skills: Basic computer skills; strong English communication (verbal/written); professional and cooperative team player

Common Diagnoses: Stroke, hip fractures
