Physical Therapist – Job Description
Position Summary:
The primary responsibility of this role is to help patients regain optimal physical function and support the achievement of individualized therapy and medical goals within a skilled nursing facility (SNF) setting. This includes developing and implementing treatment plans, providing patient education, and promoting long-term success through home exercise programs and activity modifications post-discharge.
Qualifications:

Bachelor’s degree or higher in Physical Therapy from an accredited college or university

Active, unrestricted state license to practice Physical Therapy

Basic computer proficiency, including the ability to use therapy documentation systems and electronic health records
