Job Description

American Traveler is seeking a Home Health Physical Therapist with home health certification and proficiency in OASIS for patient care in the community.

Responsibilities

• Home health setting providing direct patient care in clients' homes
• Works with rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, ADL equipment, and modalities
• Independent work with minimal supervision required
• Requires travel between patient homes
• Primary responsibilities include direct patient care, collaboration with team members, and participation in developing comprehensive plans of care
• Must be able to work independently and manage cases with minimal supervision
• Candidates who reside within 50 miles of the facility will not be considered
• A maximum of 5 days of requested time off will be considered

Requirements

• Certified Home Health experience required
• Proficiency with OASIS documentation is required
• Minimum 2 years of acute nursing care (hospital base) and home care experience
• Current Massachusetts physical therapy license required if applicable
• Current BLS certification (AHA) required
• Valid driver’s license required
• Acute care experience preferred
