Description

American Traveler is seeking a Physical Therapist for a home health assignment, requiring at least 1 year of experience and active licensure.

Details

  • Home health setting with majority adult visits and occasional pediatric patients
  • Typical caseload is 5-6 visits per day, or 3 visits plus admissions on days with evaluations
  • Monday through Friday, 8:00am–5:00pm schedule with no weekends or holidays
  • May be asked to provide occasional safety evaluations or teaching visits for hospice patients
  • Utilizes Home Care Home Base EMR system
  • Must provide own transportation and maintain current driver’s license and auto insurance

Requirements

  • Active ND PT license or compact state licensure required; license must be active at time of consideration (license required if applicable)
  • Minimum 1 year of PT experience preferred, with home health and OASIS experience strongly desired
  • Current BLS certification required
  • Must have a valid driver's license and auto insurance
  • Experience with Home Care Home Base EMR preferred
  • Two professional references required: one from a supervisor within last 12 months, and one from a peer or supervisor within last 3 years

Additional Information

  • Daily responsibilities include performing start of care assessments, recertifications, resumptions of care, and regular therapy visits
  • Support resources available from sister agency therapists, clinical coordinator, and director
  • Orientation period of 1-2 weeks with preceptor support
  • Scrub colors accepted: black, navy blue, or grey
  • First-time travelers are accepted for this role
  • Must not have been employed (FT, PT, PRN) by any CommonSpirit-affiliated facility in the past 12 months
  • Candidates must live at least 50 miles from the facility
  • RTO must be declared up front; no additional time off approved after submission
