Description

American Traveler is seeking an experienced Physical Therapist for a Home Health contract role requiring recent Oasis and HCHB experience.

Details

• Home health setting with a territory up to a 60-mile radius from the main office
• Geriatric patient population with an average caseload of 5-7 patients per day
• Day shift schedule: 8:00 AM to 5:00 PM, five 8-hour shifts per week
• On-call rotation required every 3rd to 4th weekend
• Uses HCHB (Homecare Homebase) EMR; proficiency is required
• Oasis documentation required for SOC (Start of Care) visits

Requirements

• Active Physical Therapy license required if applicable
• Minimum 1 year of recent home health experience required
• Proficient Oasis and HCHB experience required
• Current BLS certification required
• Flexibility with coverage area is required
• Timely documentation and communication are essential

Additional Information

• Candidates must wear scrubs in any color; jeans are not permitted
• Travelers will be provided with an iPad for documentation and mileage is reimbursed at the IRS standard rate
• Strong emphasis on timely and accurate record keeping and professional communication
• Applicants must be comfortable with independent work and managing daily scheduling
• Local candidates are accepted at the travel rate
• RTO (requested time off) must be disclosed at the time of application
