Job Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health and OASIS experience.

Responsibilities

• Home health setting providing one-on-one physical therapy to patients in their homes
• Monday through Friday schedule with weekend rotation
• Role includes case management responsibilities
• Uses McKesson/Netsmart EMR, with experience strongly preferred
• Responsible for developing and implementing individualized care plans to maximize patient mobility and independence
• Collaboration required with interdisciplinary teams to ensure comprehensive patient care
• Utilization of a variety of therapeutic modalities and standard assessment techniques, including ROM, strength, and balance tests
• Scrub or business casual attire accepted for in-home patient care
• Potential for consideration of local candidates as no radius rule is indicated

Requirements

• Active Vermont PT license required if applicable
• Minimum 1 year of home health physical therapy experience required
• Demonstrated OASIS experience required for consideration
• Current ACLS and BLS/BCLS certifications required
• PALS and NRP certifications are listed as general certifications
• Rehabilitation-specific certifications such as ASHA, CHT, or ACP Training noted
• Strong ability to perform and document standard therapy measurement techniques
