Job Description

American Traveler is seeking a Physical Therapist for a home health position serving adult and geriatric populations, requiring Wisconsin licensure and recent home health experience.

Responsibilities

• Home health setting with coverage across central and northern regions as needed
• Primary work schedule is Monday through Friday, 8:00 AM to 5:30 PM
• Physical Therapists rotate one Saturday every 6-8 weeks, with an alternate weekday off
• Patient population is primarily adult and geriatric with orthopedic, neurological, and functional mobility diagnoses
• Productivity expectation is an average of 4.5 visits per day
• Laptop provided for point of service documentation using NetSmart EMR
• Cell phone provided for GPS and communication
• Mileage is reimbursed at the government-approved rate
• Duties include evaluation, treatment, and planning of patient care in accordance with Physical Therapy standards and regulations
• Must document care to meet regulatory and reimbursement guidelines
• Responsible for meeting established productivity standards
• Resilience, adaptability, and strong social skills are valued for this position
• Scrub attire in any color or business casual dress is acceptable
• Option to be placed on call for low census hours; up to 36 unbillable low census hours per contract
• Contractors must live outside a 50-mile radius from the facility
• No holiday coverage required

Requirements

• Current licensure as a Physical Therapist in Wisconsin required; license required if applicable
• AHA BLS certification required
• Master's Degree in Physical Therapy required
• NPI number required or willingness to apply prior to starting
• Recent home health experience is required
• Training and experience working with adult and geriatric patients with various diagnoses
• Strong documentation skills and ability to use NetSmart EMR
