Job Description

American Traveler seeks a Physical Therapist for Home Health with at least 1 year of recent experience and expertise in OASIS and HCHB.

Responsibilities

• Home health setting serving a geriatric patient population
• Patient load averages 5-7 visits per day with confirmation during interview
• Service area includes up to a 60-mile radius from the central office
• Day shifts, Monday through Friday, from 8:00 am to 5:00 pm
• On-call rotation required every third or fourth weekend
• Documentation utilizes HCHB (Homecare Homebase) EMR and HCHB experience is required
• iPad provided for work use; no cell phone provided
• Mileage reimbursed at the IRS standard rate
• Scrubs of any color are required; jeans are not permitted
• Travelers must be flexible within the assigned territory
• First-time travelers are not eligible for this assignment
• Requested time off must be submitted at the time of application
• Former staff must have a minimum six-month separation and HR approval to return as travelers
• Travel pairs are accepted, but each traveler will be assigned an individual territory and must have their own transportation

Requirements

• Active PT license required if applicable
• Minimum 1 year of recent home health physical therapy experience required
• OASIS experience required for those completing Start of Care
• Current BLS certification required
• HCHB (Homecare Homebase) proficiency required
• Active driver’s license and personal transportation required
• Timely documentation and communication skills are mandatory
• Completion of Hospice/Home Health Checklist required for consideration
