Job Description

American Traveler is seeking a Physical Therapist for a Home Health position serving East Portland, OR, and surrounding areas, requiring at least 1 year of skilled home health experience and BCLS certification.

Responsibilities

• Home health position with 1:1 ratios and a typical caseload of up to 6 patient visits per day
• Service area includes East Portland, OR, and surrounding areas within a maximum radius of about 60 miles or 60-90 minutes driving time per day
• Standard shift is Monday through Friday, day shift, 8-hour shifts from 8:00 to 16:30
• No weekend, on-call, or block scheduling required
• No floating between like units required within the assignment, though travelers are expected to float between all facilities within the state if needed
• Uses Epic Remote Client for Home Health charting on laptop and Rover phone app, along with Oasis documentation
• Mileage is reimbursed at IRS standard rate and free parking is available
• Responsibilities include providing skilled physical therapy care to patients in their homes and documenting visits using Epic and Oasis
• System groups client visits to minimize driving and windshield time
• Business casual attire is required; scrubs are allowed but not required, and jeans are not permitted
• First-time travelers are accepted for this role
• Former permanent staff must wait at least 3 months before returning as a traveler and cannot be within 50 miles of the facility
• Minimal requested time off is permitted and is subject to hiring manager approval; all RTO must be submitted during initial contracting
• Local candidates are accepted at the same rate if residing within a 50-75 mile radius

Requirements

• Active PT license required if applicable; pending license accepted
• Minimum of 1 year of recent skilled home health experience required
• Current BCLS certification required
• OASIS experience required; candidates without OASIS experience must clearly note this
• Must have own transportation for daily client visits
• Primary series COVID vaccination plus one booster required
• SSN and date of birth required for consideration
• At least two supervisory references covering one year of employment within the last three years in a comparable specialty or setting
