Job Description

American Traveler is seeking a Physical Therapist with at least one year of home health experience for a flexible day shift assignment in the San Jose area.

Responsibilities

• Home health position providing physical therapy services in patient homes
• Covers a service area including Milpitas, Saratoga, Monte Sereno, Los Gatos, Campbell, San Jose, Morgan Hill, San Martin, Gilroy, Coyote Point, Sunnyvale, and neighboring cities
• Day shift from 8am to 5:30pm with flexible scheduling based on patient availability
• Requirement to work one weekend day per month
• Responsibilities include evaluating and treating patients based on prescribed physical therapy plans
• May supervise Physical Therapy Assistants during patient care
• Responsible for documenting clinical findings, problems, interventions, goals, and discharge plans
• Conducts initial admission and evaluation visits, and communicates OASIS scores to the Clinical Team Manager within set timelines
• Other duties may be assigned in alignment with professional skills and organizational mission

Requirements

• Active state physical therapy license required if applicable
• Minimum of 1 year of recent home health physical therapy experience
• Current BLS or BCLS certification required
