Job Description

American Traveler is seeking a Physical Therapist with at least 1 year of recent home health experience for a flexible day shift position.

Responsibilities

• Home health setting covering territories in Atherton, Redwood City, Belmont, San Mateo, Burlingame, Milbrae, San Bruno and nearby areas
• Work schedule is typically 8am-5:30pm with flexibility based on patient availability
• Required to work one weekend day per month
• Will evaluate and treat patients in their homes using prescribed physical therapy programs
• Responsibilities include evaluating, planning, and administering medically prescribed physical therapy for clients in a home health environment
• Expected to document observations, clinical findings, interventions, goals, and discharge plans
• May supervise Physical Therapy Assistants and make initial admission or evaluation visits
• Must notify the assigned Clinical Team Manager of OASIS scores within established timelines
• Position involves travel between patient homes within assigned territories

Requirements

• Current and active state license required if applicable
• Minimum 1 year of home health physical therapy experience required
• Current BLS/BCLS certification required
