Job Description

American Traveler seeks a Physical Therapist with at least 1 year of home health experience and BLS certification for a day shift home health position.

Responsibilities

• Position is based in home health, covering a range of conditions including med/surg, cardiac, ortho, post-surgical, respiratory, neurological, and trauma cases
• Typical caseload is 6-8 skilled home visits per day with visits and hours self-scheduled depending on office needs
• Patients include adults recovering from surgeries, orthopedic injuries, strokes, neurological conditions, chronic illnesses, and trauma
• Uses BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation system
• Work schedule is day shift (5x8 hours), 8:00 - 17:00, with 1-2 weekends per contract
• Home visits require valid driver’s license and personal vehicle
• Equipment provided includes laptop, MiFi, and portable printer
• Duties include conducting skilled assessments, implementing home exercise programs, performing wheelchair and equipment assessments, and providing patient and caregiver education
• Responsible for safety and injury prevention, medication monitoring, and recognizing signs and symptoms of abuse
• Handles case documentation, billing, and scheduling using provided electronic systems
• Solid color scrubs are required for this position
• Work is primarily independent with responsibility for self-scheduling based on patient needs and office assignments

Requirements

• Current state PT license required if applicable
• Minimum 1 year experience in home health physical therapy required
• Current BLS certification required
• Experience with assessment, cardiopulmonary and circulatory conditions, documentation, and patient family education
• Proficiency using devices such as gait belts, rolling walkers, sit-to-stand lifts, sliding boards, and supplemental oxygen equipment
• OASIS training preferred
• Must be COVID vaccinated or provide exemption documentation
• Must have current flu shot or submit declination with mandatory mask usage
• Valid driver’s license and reliable transportation required
