Job Description

American Traveler is seeking a Physical Therapist for home health and hospice patients, requiring VT state licensure and experience working independently.

Responsibilities

Home health and hospice care environment
Monday through Friday work schedule
Provides in-home physical therapy services
Collaborative multidisciplinary team setting
Main responsibilities include maximizing independence and safety for home care and hospice patients through therapy
Supports and collaborates with a multidisciplinary care team
Role involves traveling to patient homes within the service area
Open to local candidates

Requirements

Active Vermont Physical Therapist license required if applicable
Valid driver’s license and reliable transportation required
Experience in a home care setting is preferred
Ability to work both independently and as part of a team
