Job Description

American Traveler is seeking a Physical Therapist for home health and hospice patients, requiring VT state licensure and experience working independently.

Responsibilities

• Home health and hospice care environment
• Monday through Friday work schedule
• Provides in-home physical therapy services
• Collaborative multidisciplinary team setting
• Main responsibilities include maximizing independence and safety for home care and hospice patients through therapy
• Supports and collaborates with a multidisciplinary care team
• Role involves traveling to patient homes within the service area
• Open to local candidates

Requirements

• Active Vermont Physical Therapist license required if applicable
• Valid driver’s license and reliable transportation required
• Experience in a home care setting is preferred
• Ability to work both independently and as part of a team
