Job Description

American Traveler is seeking a Physical Therapist with at least one year of Home Health experience for a day shift position covering communities in Vermont and New Hampshire.

Responsibilities

• Home health setting serving communities in VT and NH
• Day shift position
• Service area includes White River Junction, Lebanon, Bradford, and Windsor regions
• Care provided to patients in their residences with 45 minute average visit lengths
• Therapist functions with both independence and as part of an interdisciplinary care team
• Physical Therapists evaluate patients and develop individualized treatment plans
• Treatments and plans must align with agency policies and professional standards
• Effective communication and coordination with team members are essential
• Responsibilities include teaching patients and families about treatments and procedures
• Travel between several towns is required within the service area for patient visits
• Documentation of evaluations, goals, and outcomes must be thorough and timely

Requirements

• Current PT license in VT and NH or unencumbered compact license required if applicable
• Minimum of one year of physical therapy experience in a home health setting
• Bachelor’s degree in Physical Therapy required
• Current BLS/BCLS certification required
• Rehabilitation certifications such as ASHA, CHT, or ACP Training are considered
