Job Description

American Traveler seeks a Home Health Physical Therapist with at least 1 year of recent home health experience and a current state license for day shifts across multiple counties.

Responsibilities

• Home health setting with patient visits across designated counties including Sonoma, Marin, Contra Costa, Alameda, Santa Clara, San Joaquin/Stanislaus, and Sacramento
• Caseload emphasizes patient-centered care with a minimum of 23-24 visits per week to qualify for full-time hours
• Geriatric, wound care, and other home health cases are typical
• Day shifts, Monday through Friday, 8:00 AM to 5:00 PM
• No weekend or mandatory on-call requirements; on-call may be discretionary
• Mileage reimbursed at IRS standard rate from first to last patient
• Kinnser/Wellsky is used for charting, and all charting must be completed at the clinic site
• Assignments may require coverage of surrounding counties as needed
• Responsibilities include providing home health physical therapy, wound care, and geriatric patient management
• Patient-focused, punctual, self-starter clinicians are especially valued
• No mandatory weekend or on-call coverage is expected
• Free residential parking is available
• Any color scrubs with closed-toe shoes are acceptable for dress code
• COVID vaccination required, with declination possible for religious reasons; flu vaccination may be declined with proper documentation
• Orientation to counties and specific coverage areas will be discussed during interview
• Four- to six-month contract lengths available; six months required for clinicians with less experience
• First-time travelers are encouraged to apply
• Role accepts local candidates within a 50-mile radius

Requirements

• Active state PT license required at time of consideration; license required if applicable
• Minimum 1 year of home health experience and 2 years total clinical experience
• Current BLS certification required
• No adverse license actions ('hits') permitted
• All required compliance documents must be completed before start
• Ability to complete charting on-site using a provided tablet
• Must submit all requested time off at the time of consideration; no additional RTO accepted after offer
