Description

American Traveler is seeking a Physical Therapist with home health experience and BLS certification for a day shift position in a skilled home care environment.

Details

• Home health setting serving Medicare, Medicare Advantage, and commercial insurance patients
• Patient population includes med/surg, cardiac, orthopedic, neuro, post-surgical, COPD, CHF, pneumonia, stroke, trauma, Parkinson's, and respiratory cases
• Typical caseload is 6-8 skilled home visits per day, with a minimum of 6 points per day and possible increase at manager’s discretion
• Day shift schedule from 8:00 to 17:00, 5 days per week
• 1-2 weekend shifts required per contract
• Self-scheduling of visits and hours based on office needs
• Required equipment provided, including laptop, MiFi for internet, and portable printer
• Uses BayCare Forms, BayCare Scheduling system, and Ace Timesheet documentation

Requirements

• Current state PT license required if applicable
• BLS certification required
• Minimum 1 year of recent physical therapy experience, including home health
• OASIS training preferred
• Must possess valid driver's license and reliable car for home visits
• COVID-19 vaccination or approved exemption required
• Flu vaccination or declination with mandatory masking required

Additional Information

• Main responsibilities include assessment, care planning, documentation, equipment use, implementation of home exercise programs, and patient/family education
• Experience with safety and injury prevention, billing, and identification of various forms of abuse is required
• Patient care includes use of gait belts, walkers, lift devices, wheelchairs, prosthetics, collars, supplemental O2, and adaptive equipment
• Daily documentation required using provided systems
• Solid color scrubs are required
• Open to travel contracts of at least 13 weeks with potential for extension
