Job Description

American Traveler is seeking a Physical Therapist for a home health position serving a rural territory, requiring experience with HCHB and an active PT license.

Responsibilities

• Home health setting covering Cleveland, Gaston, and Rutherford counties
• Geriatric patient population with common visits, evaluations, admissions, and discharges
• Average caseload of 6 patients per day
• Team includes 2 PTs, 1 PT in orientation, and 1 PTA
• Day shift schedule from 8:00 AM to 5:00 PM, Monday through Friday
• No weekend, holiday, or on-call requirements
• Electronic medical record system used is Homecare Homebase (HCHB)
• Main responsibilities include patient assessments, developing care plans, performing admissions and discharges, and providing direct home-based therapy to geriatric patients
• A five-day orientation is provided for onboarding
• Team members include experienced staff and a supportive interdisciplinary environment
• Scrub color required is light gray
• Holiday coverage is as needed
• RTO requests must be approved by the manager
• Disclosure of any board action or criminal history is required for consideration
• Primary source license verification is required for each state with active licensure

Requirements

• Active Physical Therapist (PT) license required; license required if applicable
• Current BLS certification
• Minimum 12 months of recent core experience in the unit specialty area, with experience within the past year
• Valid car insurance required
• Experience with Homecare Homebase (HCHB) EMR preferred
• One professional supervisory reference from the past year required
