Job Description

American Traveler is seeking a Physical Therapist with at least one year of home health experience for a flexible day shift assignment.

Responsibilities

• Home health care position serving a diverse geographic territory including San Jose and surrounding areas
• Work schedule is typically 8am to 5:30pm with flexibility based on patient availability
• Requires one weekend day per month
• Patient caseload and visits are determined by home health needs and agency assignment
• Responsibilities include evaluating and treating patients per prescribed physical therapy programs
• May supervise Physical Therapy Assistants and conduct initial admission and evaluation visits
• Responsible for timely documentation of clinical findings, interventions, goals, and discharge plans
• Must communicate assessment scores to the clinical team manager according to agency timelines
• Role may require covering additional neighboring cities as needed to meet patient care demands

Requirements

• Minimum of one year home health physical therapy experience required
• Active state license required if applicable
• Current BLS or BCLS certification required
• Ability to travel throughout a multi-city regional territory
