Job Description

American Traveler seeks a Physical Therapist with 2+ years of recent home health experience and an active CA PT license for a day shift home health assignment.

Responsibilities

Home health setting serving the San Jose, South Bay, and Santa Clara County areas
Day shift schedule with 5x8 hour shifts from 8:00 AM to 5:00 PM
Patient care is delivered in the patient's home environment
Must be available to go for lab work within 3-5 days of signing ACA
Only 1 holiday request for time off (RTO) will be approved
Responsibilities include delivering PT services, documenting home health visits, and managing caseloads across designated geographic areas
Submission consideration requires recent supervisor references and copies of all certifications

Requirements

Current and active CA PT license required
Current BLS certification required
Minimum 2 years of recent physical therapy experience with at least some in home health
Must meet the 50-mile radius rule for eligibility
Return staff must have been gone at least 1 year to be eligible
License required if applicable
