Description

American Traveler is seeking a Physical Therapist with at least 1 year of recent home health experience, Oasis and HCHB proficiency, for a home health role covering the Suffolk area.

Details

Home health position serving geriatric patients in Suffolk and surrounding ZIP codes within a 60-mile radius
Typical caseload is 5-7 patients per day, subject to confirmation during interview
Day shift schedule from 8:00 am to 5:00 pm, Monday to Friday
Electronic documentation using HCHB EMR system
No on-call requirement
Flexible geographic assignment within designated territory
Locals are accepted at the travel pay rate

Requirements

Current and active Physical Therapist license required if applicable
Minimum of 1 year recent home health experience
BLS certification required
Oasis clinical assessment experience required
Proficiency with HCHB EMR required
Ability to travel up to a 60-mile radius from the hub or office

Additional Information

Responsible for timely documentation and effective communication with the care team
Candidates must be flexible with adapting to varying patient assignments within the territory
First-time travelers are welcome
Mileage is reimbursed at the IRS standard rate
An iPad is provided for documentation; cell phones are not provided
Scrubs of any color are required; jeans are not permitted
