Job Description

American Traveler is hiring a licensed Occupational Therapist for a home health position in the Grand Junction, CO area, providing in-home, patient-centered occupational therapy services Monday through Friday.

Responsibilities

• Home health setting providing one-on-one, in-home occupational therapy visits to patients in the Grand Junction, CO area
• Monday–Friday, 8:00 AM–5:00 PM schedule
• Patient population includes geriatric and medically complex individuals requiring skilled OT services
• Services include ADL/IADL training, upper extremity strengthening, fine motor coordination, adaptive equipment training, energy conservation, and home safety and fall-prevention strategies
• EMR: Homecare Homebase (HCHB); proficiency preferred, willingness to learn accepted
• 13-week contract
• Perform OT evaluations, SOC visits, reassessments, recertifications, and discharge visits
• Develop and implement individualized plans of care aligned with patient goals and physician orders
• Educate patients and caregivers on adaptive techniques, home programs, and safety strategies
• Collaborate with an interdisciplinary team including RNs, PTs, STs, and OTAs
• Provide clinical oversight of OTAs when applicable and permitted under Colorado law
• Complete timely, accurate documentation in compliance with Medicare Conditions of Participation, payer requirements, and HIPAA
• DOB and SSN may be required for consideration

Requirements

• Current, unrestricted Colorado OT license required (license required if applicable)
• Graduation from an accredited Occupational Therapy program
• Valid driver's license and reliable personal transportation
• Ability to practice independently in a home-based clinical setting
• No minimum experience required
• Home health experience preferred
• Familiarity with Medicare documentation and home health regulations preferred
• Experience with geriatric and medically complex patient populations preferred
