Job Description
American Traveler is hiring a licensed Occupational Therapist for a home health position in the Grand Junction, CO area, providing in-home, patient-centered occupational therapy services Monday through Friday.
Responsibilities
- Home health setting providing one-on-one, in-home occupational therapy visits to patients in the Grand Junction, CO area
- Monday–Friday, 8:00 AM–5:00 PM schedule
- Patient population includes geriatric and medically complex individuals requiring skilled OT services
- Services include ADL/IADL training, upper extremity strengthening, fine motor coordination, adaptive equipment training, energy conservation, and home safety and fall-prevention strategies
- EMR: Homecare Homebase (HCHB); proficiency preferred, willingness to learn accepted
- 13-week contract
- Perform OT evaluations, SOC visits, reassessments, recertifications, and discharge visits
- Develop and implement individualized plans of care aligned with patient goals and physician orders
- Educate patients and caregivers on adaptive techniques, home programs, and safety strategies
- Collaborate with an interdisciplinary team including RNs, PTs, STs, and OTAs
- Provide clinical oversight of OTAs when applicable and permitted under Colorado law
- Complete timely, accurate documentation in compliance with Medicare Conditions of Participation, payer requirements, and HIPAA
- DOB and SSN may be required for consideration
Requirements
- Current, unrestricted Colorado OT license required (license required if applicable)
- Graduation from an accredited Occupational Therapy program
- Valid driver's license and reliable personal transportation
- Ability to practice independently in a home-based clinical setting
- No minimum experience required
- Home health experience preferred
- Familiarity with Medicare documentation and home health regulations preferred
- Experience with geriatric and medically complex patient populations preferred
