Job Description

American Traveler is seeking a licensed Physical Therapist with home health experience for a 13-week travel contract serving patients across San Francisco and surrounding communities.

Responsibilities

• Home health setting serving patients across San Francisco, Brisbane, Daly City, Colma, South SF, Pacifica, Millbrae, Hillsborough, Burlingame, and neighboring cities
• Day shift hours from 8:00 AM to 5:30 PM, with schedule flexibility based on patient availability
• 40-hour work week
• Responsibilities include evaluating and treating patients under medically prescribed physical therapy programs, with a focus on restoring function and maximizing patient performance
• Performs initial admission and evaluation visits, including OASIS score documentation within established timelines
• May supervise Physical Therapy Assistants as part of the care team
• Documents clinical findings, skilled interventions, goals, and discharge plans in accordance with home health standards
• Reports OASIS scores to assigned Clinical Team Manager within required timelines

Requirements

• Minimum 1 year of home health PT experience required
• Current, active state PT license required
• Current BLS/BCLS certification required
• License required if applicable
