Description

American Traveler is seeking a Physical Therapist with at least 3 years of experience for a 13-week skilled nursing facility assignment focused on maximizing residents' functional abilities and independence.

• Resident population includes individuals requiring physical therapy to improve functional abilities and independence in activities of daily living
• Day shift schedule
• 13-week contract

Requirements

• Minimum 3 years of physical therapy experience required
• Bachelor's degree in Physical Therapy required; Master's or Doctorate preferred
• Active PT state license required (license required if applicable)
• BLS/BCLS certification required
• Prior experience in a long-term care or SNF setting preferred
• Proficiency in Microsoft Office required
• Must be able to lift up to 50 lbs independently; lifts over 50 lbs require a two-person assist
• Must be able to perform a combination of sitting, standing, walking, bending, stooping, twisting, and reaching
• Strong verbal and written communication skills in English required
• Date of birth (DOB) and SSN may be required for consideration

Additional Information

• Responsibilities include evaluating residents, developing individualized plans of care with functional short- and long-term goals, and progressing treatment toward those goals
• Collaborates with physicians, PTAs, nursing staff, and the broader interdisciplinary care team, including attendance at relevant team meetings
• Provides resident and caregiver education on therapeutic exercises, assistive devices, and home programs
• Documents all evaluative and therapeutic interactions in the EMR in accordance with Medicare, Medicaid, and other reimbursement requirements
• Assists with in-service education for nursing and other departments, and contributes to program development within the Rehabilitation department
• Mentors new staff during orientation and supervises assistant-level staff in compliance with state mandates and professional standards
• Responsible for maintaining department cleanliness and adhering to infection control procedures
