Description

American Traveler is seeking a Physical Therapist for a home health position requiring at least 1 year of home health experience, with daily patient visits across a multi-city service area in North Carolina.

Details

• Home health setting serving adult patients across Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and surrounding cities
• Patient population includes adults with chronic diseases and orthopedic/post-surgical conditions
• Caseload of 6 patient visits per day with a required 30 visit equivalents per week
• Average driving distance is 60–80 miles per day; mileage is reimbursed weekly
• 12-hour day shifts (0800–2000), 36 hours per week
• No weekends, no on-call, and no major holidays required
• Therapists schedule patient visits the day prior to the planned visit
• EMR systems include Epic Rover (view only), Focura, and Home Care Home Base
• A company cell phone (Samsung Note 9) is provided for documentation and patient/provider communication

Requirements

• Minimum 1 year of home health experience required, with most recent experience within the last year
• Valid PT state licensure required; license required if applicable
• Current BLS certification required
• Valid driver's license and proof of auto insurance required
• Candidates must reside at least 50 miles from the home unit to be eligible
• At least 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN
• Last 4 digits of SSN and full date of birth required for consideration

Additional Information

• A manager interview is required as part of the selection process
• 5-day on-site orientation provided
• Cancelled appointments do not count toward weekly visit equivalent requirements
• Grey scrubs required
• Supported by a clinical manager, clinical supervisor, scheduler, and peer clinical team
• RTO requires manager approval
• First-time travelers will be considered if they have a strong home health background
• Candidates who have worked a permanent position at an Advocate Health Southeast (AHSE) facility must have been separated for at least 1 year before being eligible
