Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of experience and OASIS knowledge for a 13-week weekday-only assignment in West Virginia.

Details

• Home health setting with patient visits conducted in the community/home environment
• Schedule is Monday through Friday, 8:30 AM - 5:00 PM, with no weekends or on-call requirements
• EMR system is Netsmart (McKesson)
• Mileage is reimbursed at the IRS standard rate

Requirements

• Minimum 1 year of recent home health PT experience required
• OASIS knowledge required
• BLS certification required
• WV State PT License required if applicable
• Proficient in home health documentation including assessments, evaluations, plan of care, OASIS, RUG levels, short/long-term goals, reassessments, and discharge planning
• Strong time management skills required
• Two professional references required for consideration

Additional Information

• Preferred skills include assessment of adaptive/assistive devices, body mechanics, environmental barrier identification, pain assessment, and strength testing
• Preferred equipment experience includes gait belt, hospital bed, sit-to-stand lift, and wheelchair assessment
• Preferred orthopedic experience includes gait training, extremity mobilization, hip fractures, osteoarthritis, spinal conditions, total hip and knee replacement
• Preferred neurologic experience includes post-stroke rehabilitation
• Preferred professional skills include home exercise program development and patient/family education
• Flu vaccine is required; medical exemptions are considered with a signed physician's note and completion of the facility's exemption form — religious exemptions are not accepted
