Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of recent experience and required OASIS and HCHB EMR proficiency for a 13-week travel contract in Portage, MI.

Details

• Home health setting serving a geriatric patient population
• Caseload of approximately 5-7 patients per day (to be confirmed during interview)
• Territory covers up to a 60-mile radius from the hub/office
• Day shift schedule, 5x8-hour shifts (8:00 AM – 5:00 PM)
• On-call required every 3rd to 4th weekend
• HCHB used for documentation; experience required
• iPad provided for clinical use

Requirements

• Minimum 1 year of recent home health PT experience required
• OASIS experience required
• HCHB EMR experience required
• Current BLS certification required
• PT license required if applicable
• Must be flexible with territory coverage within the designated service area
• Ability to maintain timely documentation and communication

Additional Information

• Primary responsibilities include evaluation and treatment of geriatric home health patients across an established geographic territory
• Mileage reimbursed at the IRS standard rate
• Scrubs required in any color; jeans are not permitted
• Local candidates are accepted and eligible for the travel pay rate

• 2 professional supervisory references from within the past 2 years required for consideration
• Full 7-year work history with all gaps explained required for consideration
