Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of recent experience, OASIS and HCHB proficiency, and flexibility to cover a wide service territory in Alpena, MI.

Details

Home health setting serving a geriatric patient population
Caseload of approximately 5-7 patients per day (to be confirmed during interview)
Service territory covers up to a 60-mile radius from the hub/office
Day shift schedule, 5x8-hour shifts (8:00 AM - 5:00 PM)
On-call required every 3rd to 4th weekend
EMR: HCHB (experience required)
iPad provided for documentation in the field

Requirements

Minimum 1 year of recent home health PT experience required
OASIS experience required
HCHB EMR experience required
Current BLS certification required
Active PT license required if applicable
Must be flexible with service territory coverage

Additional Information

First-time travelers are welcome to apply
Local candidates are accepted and eligible for the travel rate
Timely documentation and prompt communication are expected
Scrubs required in any color; jeans are not permitted
Mileage is reimbursed at the IRS standard rate
Personal cell phone is not provided; an iPad is supplied for field use
All time-off requests must be submitted at the time of initial consideration
2 professional supervisory references from within the past 2 years required for consideration
Full 7-year work history with all gaps explained required for consideration
