Description

American Traveler is seeking a licensed Physical Therapist (PT) for a Home Health position requiring at least 1 year of PT experience and an active SC or compact license.

Details

• Home health setting with patient visits conducted in the home environment
• Monday–Friday, 8:00 AM–5:00 PM schedule with no on-call requirements
• 40 hours per week, 8 hours per day
• Primary charting system is Cerner; experience with Homecare Homebase is preferred
• Position requires operation of a personal vehicle for patient visits

Requirements

• Active SC Physical Therapist license or compact state license (license required if applicable)
• Current AHA BLS for Healthcare Providers certification required
• Graduate of an accredited Physical Therapy program
• Minimum 1 year of experience as a Physical Therapist
• Home Health experience preferred
• Experience with Homecare Homebase preferred
• Valid, current driver's license required
• Demonstrated knowledge of therapeutic tasks, rehabilitative activities, and orthotic/prosthetic/assistive devices
• Strong organizational and time management skills
• English proficiency (speak, read, and write) required

Additional Information

• Responsibilities include evaluating, planning, and providing direct hands-on care to patients with deficits in self-care, mobility, safety, range of motion, or strength
• Responsible for supervising and directing care provided by Physical Therapy Assistants and Home Health Aides
• Coordinates with other disciplines, agencies, and families to ensure patient care needs are met
• Documents all care, coordination of services, orders, and goals in the electronic medical record accurately and in a timely manner
• Candidates must live more than 50 miles from the facility to be eligible; local candidates will not be considered
• Travelers may float to a nearby sister facility
• Travelers may remain on assignment for up to one calendar year from the initial start date; a 30–60 day break is required before returning to a new assignment
• Two references are required for consideration
