Description

American Traveler is seeking a Home Health Physical Therapist with certified home health experience and OASIS proficiency for a travel assignment in Massachusetts.

Details

• Home health setting providing physical therapy services to patients in their place of residence
• Rehabilitation equipment utilized includes splints, orthoses, prostheses, wheelchairs, ADL equipment, and modalities
• Position requires independent work with minimal supervision

Requirements

• Minimum 2+ years of acute care (hospital-based) and home health experience required
• Certified home health experience with proficiency in OASIS assessments required and non-negotiable
• Home Health Certification required
• Valid driver's license required; must have personal vehicle for travel to patient locations
• MA PT license required if applicable
• Active AHA BLS certification required
• PT Skills Checklist current within 1 year required for consideration
• 1 supervisory signed reference from within the past 2 years of employment required for consideration

Additional Information

• Responsibilities include direct patient care, collaboration with interdisciplinary team members, and participation in developing each patient's comprehensive plan of care
• Candidates must reside more than 50 miles from the facility to be eligible for this position
• All RTO requests must be submitted at the time of application; requests made after an offer is extended may result in a rescinded offer
• No more than 5 days of RTO may be requested
