Description

American Traveler is seeking a Physical Therapist (PT) with 2+ years of experience for a home health position serving a primarily orthopedic and geriatric patient population across Tehama County.

Details

• Home health care setting serving most of Tehama County
• Average daily census of 78 patients, expected to grow with additional PT availability
• Patient population includes orthopedic/joint replacement and geriatric patients with multiple comorbidities such as CHF and COPD
• PT staff conduct Start of Care (SOC) visits, particularly for orthopedic patients when nursing is not ordered
• Patient caseload of approximately 5 visits per day
• Daily driving is approximately 40 miles, expected to decrease as patient load is redistributed
• EMR: Netsmart (laptop provided for charting)
• 8-hour day shifts, with occasional Saturday coverage for orthopedic patients
• Holidays off
• No on-call or ventilator responsibilities

Requirements

• Minimum 2 years of physical therapy experience required
• Current PT license required; pending licenses will not be accepted — license required if applicable
• Active BLS certification required; pending certifications will not be accepted
• Valid driver's license required for consideration
• 2 professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, both including dates of employment and eligibility for rehire

Additional Information

• Responsibilities include home health evaluations and treatment visits, with a focus on orthopedic and geriatric patients
• The home health therapy team includes PT, OT, and SLP disciplines, with intake RN and manager support in the office
• Nurse aides are available to support the care team
• 12-hour facility orientation provided
• Candidates who have been directly employed (full-time, part-time, or PRN) by any CommonSpirit, CHI, or Dignity Health facility within the past year are not eligible; candidates employed PRN through outside firms will be considered
