Description

American Traveler is seeking an experienced RN House Supervisor for a skilled nursing facility in Rochester, NY, requiring prior charge experience and an active NY RN license.

Details

• Skilled nursing facility (SNF) setting with an average daily census of 180 residents across 5 distinct units
• Units include a neurobehavioral dementia/Alzheimer's unit (with residents exhibiting violent behaviors), a secure dementia unit for elopement-risk residents, a rehab unit for post-orthopedic and cardiac patients, and two long-term care units
• Resident diagnoses include dementia, Alzheimer's, diabetes (DM), CVA, cardiac conditions, and orthopedic post-surgical care
• Specialized equipment in use includes wound vacs, PICC lines, nephrostomy tubes, tube feeds, indwelling catheters, and colostomies
• Epic EMR — experience preferred
• Schedule consists of three 8-hour shifts and one 12-hour shift per week (36 hours total)
• Shift times range between 11p–7a (8 hrs), 3p–11p (8 hrs), and 7p–7a (12 hrs) — primarily evenings, nights, and weekends
• Every other weekend coverage required

Requirements

• Active NY RN license required
• Current BLS certification required
• Minimum 2 years of RN experience required
• Prior charge or supervisory nursing experience required
• Skilled nursing facility experience required
• Proficiency in code management, de-escalation techniques, and managing assaultive behavior
• Experience with PICC line care and management, IV insertion and maintenance, and new admissions processes
• Knowledge of regulatory requirements including DPH, TJC, and OSHA standards
• Experience with centralized staffing, productivity standards, and coaching staff to performance standards
• Familiarity with HIPAA, FMLA, and federal/state scope of practice labor laws
• Candidates must reside more than 75 miles from the facility to be eligible
• Two professional supervisor references from within the past 2 years required for consideration
• Full 7-year work history with all gaps explained required for consideration

Additional Information

• Serves as the house-wide nursing supervisor during off-shifts (evenings, nights, weekends, and holidays) across all five units
• Acts as a clinical resource for problem solving, staff mentoring and coaching, and facilitating patient flow throughout the facility
• Responsible for organizational escalation, incident reporting, sentinel event monitoring, infection control, and workplace safety oversight
• Scrubs of any color are acceptable
