Description

American Traveler is seeking a Home Health Physical Therapist with OASIS, home health, and hospice experience for a 13-week travel contract in Vermont.

Details

Home health and hospice setting with patient-to-patient home visits
Monday through Friday, 8:00 AM – 5:00 PM schedule
Holiday coverage required as needed
Uses HealthWyse EMR

Requirements

Minimum 2 years of physical therapy experience required
OASIS experience required
Home health and hospice experience required
Must have experience traveling between patient homes for in-home visits
Active Vermont PT license required if applicable
Must be able to manage and adapt to schedule changes

Additional Information

Responsibilities include evaluation and treatment of patients in a home health and hospice environment
Must be flexible and self-directed given the nature of home visit scheduling
Facility conducts its own background check
