Description

American Traveler is seeking a licensed Physical Therapist with home health experience for a skilled home care contract in the Lakeland, FL area.

Details

• Home health setting providing skilled visits to patients in their residences across the Davenport/Haines City and Lake Wales/Winterhaven areas
• Patient population includes med/surg, cardiac, ortho (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trach, trauma, AKA/BKA, and ADL patients
• Skilled home care visits covered under Medicare, Medicare Advantage, and commercial insurance
• Caseload of 6–8 visits per day, with a minimum of 6 points per day (may increase at manager's discretion)
• Day shift, 5x8-hour schedule (8:00–17:00); visits and hours are self-scheduled based on office needs
• 1–2 weekend shifts required per contract
• Charting done via BayCare Forms, BayCare Scheduling, and ACE Timesheet documentation system
• Equipment provided includes a laptop, MiFi for internet access, and a portable printer

Requirements

• Minimum 1 year of physical therapy experience required; home health experience required
• OASIS training preferred
• Current BLS certification required
• Valid state PT license required (license required if applicable)
• Valid driver's license and personal vehicle required for home visits
• Proficiency in assessment, cardiopulmonary/circulatory care, documentation, ortho, neuro, home exercise program development, patient/family education, safety and injury prevention, and billing
• Experience with gait belts, rolling walkers, sit-to-stand lifts, sliding boards, soft/hard collars, standard walkers, supplemental oxygen, upper extremity prosthetics, and wheelchair assessment
• COVID vaccination required, or exemption form on file
• Flu shot required; unvaccinated candidates must wear a mask at all times while on duty
• 2 professional references from a supervisor within the past 2 years required for consideration

• Responsibilities include skilled PT evaluations and treatments across a diverse home health patient population
• Any solid color scrubs are acceptable
