Description

American Traveler is hiring an experienced RN for a Long-Term Care House Supervisor position in Rochester, NY, requiring prior charge experience and the ability to oversee a 180-bed, 5-unit skilled nursing facility across evening, night, and weekend shifts.

Details

  • Long-term care skilled nursing facility with an average daily census of 180 residents across 5 distinct units
  • Facility units include a neurobehavioral dementia/Alzheimer's unit, a rehab unit, a secure dementia/elopement-risk unit, and two traditional long-term care units
  • Resident diagnoses include dementia, Alzheimer's disease, diabetes, CVA, cardiac conditions, and post-orthopedic surgery rehabilitation
  • Role functions as House Supervisor covering all 5 units during off-shifts, including evenings, nights, weekends, and holidays
  • Weekly schedule consists of three 8-hour shifts and one 12-hour shift totaling 36 hours per week
  • Shift times rotate among 11p–7a (8 hrs), 3p–11p (8 hrs), and 7p–7a (12 hrs)
  • Every other weekend coverage required
  • Epic EMR experience preferred
  • Special equipment in use includes wound vacs, PICC lines, nephrostomy tubes, tube feeds, indwelling catheters, and colostomies

Requirements

  • Active NY RN license required
  • Current BLS certification required
  • Minimum 2 years of RN experience required
  • Prior charge or supervisory nursing experience required
  • Skilled nursing facility (SNF) experience required
  • Experience with PICC line care and management required
  • Proficiency in IV insertion and maintenance required
  • Experience with centralized staffing and productivity standards required
  • Knowledge of regulatory requirements including DPH, TJC, and OSHA preferred
  • Candidates must reside more than 75 miles from the facility to be eligible
  • 2 professional references from a supervisor within the past 2 years required for consideration
  • Full 7-year work history with all gaps explained required for consideration

Additional Information

  • Serves as the clinical resource and problem-solving authority for all 5 units during off-shift hours, facilitating patient flow and staff mentoring
  • Responsible for supervising nursing care delivered across all units organizationally, including coaching and counseling staff to performance standards
  • Manages new admissions and oversees centralized staffing during assigned shifts
  • Must be proficient in code management, de-escalation techniques, and managing assaultive behavior, particularly given the neurobehavioral resident population
  • Responsibilities include incident reporting, sentinel event monitoring and reporting, infection control, isolation procedures, and HIPAA compliance
  • Must be prepared to handle death-of-a-patient documentation and process, fitness-for-duty assessments, and organizational escalation procedures
  • Holiday coverage is expected as part of the off-shift supervisor role
  • No dress code requirement — any color scrubs are accepted
  • First-time travelers are welcome to apply
