Description

American Traveler is seeking a Home Health Physical Therapist Case Manager with at least 1-2 years of recent experience treating adult and geriatric patients in a fully remote, field-based role within the Chicago metro area.

Details

• 100% field-based home health position with no physical office; all meetings held virtually
• Two-day in-person orientation in Oak Brook, IL for EMR training and skills check-off
• Coverage area includes Chicago metro zip codes: 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614, and surrounding areas
• Patient population consists of adult and geriatric general med/surg patients
• Current team census is approximately 200 patients
• Expected caseload of 6 patient visits per day within the designated coverage area
• 1:1 patient visit model
• EMR: Epic (experience required)
• Shift hours: 8am–5pm, Monday–Friday
• No weekend rotation, no on-call, and no holiday coverage requirements

Requirements

• Minimum 1 year of recent home health PT experience required; 2 years preferred, specifically with adult and geriatric patients
• Active IL PT license required (license required if applicable)
• Current BLS certification (AHA) required
• Valid driver's license and active auto insurance required
• Reliable personal vehicle required for daily patient visits
• Experience assisting with ADLs and adaptive equipment
• Ability to effectively collaborate with interdisciplinary team members including RN, OT, and Speech specialists
• 1 professional reference dated within the last year from a Supervisor, Manager, Director, or Charge RN required for consideration

Additional Information

• Responsibilities include independent case management, patient evaluation, and treatment within the assigned home health coverage area
• Supplies and equipment are delivered directly to the traveler's home
• Supported by a nursing supervisor, manager, and team leads
• Any solid scrub color is acceptable; business casual attire is also permitted — no jeans or open-toe shoes
• Local candidates are accepted at the same rate; no radius rule applies
• First-time travelers may be considered for this position
• Former employees of Advocate Health or any of its subsidiaries must be separated for at least 12 months before being eligible
