Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of recent experience for a 13-week contract serving a geriatric patient population.

Details

• Home health setting serving a geriatric patient population
• Average caseload of 5–7 patients per day (to be confirmed during interview)
• Territory covers up to a 60-mile radius from the hub/office
• Day shift schedule, 5x8-hour shifts (8:00–17:00)
• On-call required every 3rd to 4th weekend
• EMR: HCHB experience preferred but not required
• OASIS experience preferred but not required
• iPad provided for documentation

Requirements

• Minimum 1 year of recent home health PT experience required
• Current BLS certification required
• Active PT license required if applicable
• OASIS experience preferred
• HCHB EMR experience preferred
• Must be flexible with territory coverage up to a 60-mile radius from the hub/office

Additional Information

• Timely documentation and clear communication are expected in this role
• Mileage reimbursed at the IRS standard rate
• Scrubs required in any color; jeans are not permitted
• Personal cell phone is not required for this role
