Description

American Traveler is seeking an experienced RN for an outpatient Hospice/Palliative Care position requiring a minimum of 2 years of experience and a NC or compact RN license.

Details

  • Outpatient hospice setting serving patients with a terminal prognosis of 6 months or less to live, primarily geriatric population
  • Average daily census of 63 patients with approximately 5 visits per day
  • Day shift schedule: 8-hour shifts, 8:00 AM – 5:00 PM (5x8)
  • On-call rotation covering Friday, Saturday, and Sunday evenings from 8:00 PM – 8:00 AM, shared among the RN team; weeknight call (Monday–Thursday) is covered by a designated RN with volunteer opportunities available
  • Outpatient nurses rotate approximately 2 evening on-call shifts (8:00 PM – 8:00 AM) on Saturday and Sunday every 4–5 weeks to cover PRN patient and death visit needs
  • A nurse triage service screens incoming calls per protocol and contacts the on-call nurse only if a visit is required
  • No float requirement
  • EMR: Epic Remote Client and SBAR; Epic home health/hospice experience is preferred
  • Mileage reimbursed at the IRS standard rate
  • 13-week contract minimum; longer contract extensions are of interest to the facility

Requirements

  • Active NC or compact RN license required
  • Current BLS certification required
  • Minimum 2 years of RN experience required
  • Home health/hospice and skilled nursing work setting experience required
  • End-of-life care skills required: survivorship/after-treatment care and cultural awareness
  • Comprehensive pain management skills required: WHO Analgesic Ladder, adjuvant therapy, breakthrough dosing, pediatric dosage calculation, medication tolerance, pain assessment, pharmacologic and non-pharmacological interventions, proper medication disposal, types of pain (somatic, visceral, neuropathic) and their treatments, and use of verbal and nonverbal pain scales
  • Professional knowledge required: goal setting with patients and families, intake assessment and interviewing, Medicare state regulations, patient and family education and support, and positioning/preventive care for shearing and pressure ulcers
  • Symptom assessment and management skills required: altered mental status, anorexia and cachexia, aphasia, constipation/diarrhea, nausea/vomiting, and pressure ulcer staging

Additional Information

  • Preferred end-of-life care skills include: death pronouncement, death and dying counseling, identifying spiritual needs, peri-death family care, and postmortem care
  • Preferred ethical and legal skills include: advanced directives, medical futility, medical abandonment, organ donation, palliative sedation, and withholding/withdrawing treatment
  • Preferred medication management skills include: equianalgesic opioid dosing and PCA pump management
  • Preferred phlebotomy and IV skills include: phlebotomy, starting and maintaining IVs, and care of peripheral IVs, central lines, and ports
  • Preferred professional knowledge includes: DME authorization of necessity
  • Preferred symptom management skill: ascites management
  • Epic home health/hospice experience is a plus
  • Orientation consists of 40 hours of unit-specific orientation in addition to facility-wide orientation
  • All RNs wear all navy blue scrubs; scrubs are not provided
  • A Zoom interview with the hiring manager is required prior to any offer being extended
  • Local candidates are accepted at the travel rate
  • First-time travelers are welcome to apply
  • RTO after booking is not permitted and cannot be arranged on-site; call-offs or additional time-off requests may result in contract cancellation
  • 2 professional references from a supervisor within the past 2 years are required for consideration
  • State Board of Nursing license verification and NURSYS verification are required for consideration
