Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled home health experience for a 13-week travel assignment in Medford, OR and surrounding areas.
Details
- Home health setting serving Medford, OR and surrounding areas
- Day shift, 5x8-hour schedule (8:00 AM – 4:30 PM); 4x10 and 3x12 schedules are not available
- Caseload of up to 6 patients per day with a standard productivity expectation of 30 points
- 1:1 patient ratio
- Charting via Epic and OASIS
- No weekend or on-call requirements
- Travelers are expected to float between all facilities within the state; refusal to float may result in assignment termination
- Occasional patient visits may require travel up to 50+ miles, though scheduling is designed to minimize drive time
- Local candidates are accepted; candidates within a 50–75 mile radius are considered local and are eligible at the same rate
Requirements
- Minimum 1 year of skilled home health PT experience required
- OASIS experience required
- Epic EMR experience required
- BLS certification required
- Active PT license required (pending license accepted)
- Must have reliable personal transportation
- Primary COVID-19 vaccination series plus one booster required
- At least 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required for consideration
- SSN and DOB required for consideration
- License required if applicable
Additional Information
- First-time travelers are not eligible for this position
- Former permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the submitting facility
- RTO is minimal; a few days may be approved, but significant RTO requests may impact eligibility
- Any scrub color is acceptable
- Parking is free
- Mileage reimbursement is provided at the IRS standard rate
