Description

American Traveler is hiring a travel RN Case Manager for an inpatient case management position at a Chicago, IL hospital, requiring a minimum of 3 years of acute care case management experience and an active IL RN license.

Details

  • Inpatient case management setting within a 205-bed community hospital in Chicago, IL serving an underserved population, primarily adults and geriatric patients including unhoused and low-income individuals
  • Complex case types are common, including patients with social determinants of health challenges such as housing instability and financial barriers
  • Possible unit assignments include PCU, MedSurg, and LDRP; ICU assignment is unlikely but possible
  • Typical caseload of 14–20 patients
  • Uses Epic EMR and Careport for referrals
  • Monday–Friday, 8-hour day shifts from 0800–1630, with occasional late stays
  • Every 5th weekend rotation required; when working a weekend, a Monday and Friday off are provided
  • No on-call requirements
  • Holiday coverage required for up to 2 holidays during the contract
  • Assigned to a specific unit with the possibility of floating to other inpatient units

Requirements

  • Active IL RN license required
  • ASN or higher accepted
  • Minimum 3 years of acute care case management experience required, with most recent experience within the last year
  • Proficiency with Epic EMR
  • Strong assessment, critical thinking, and interpersonal skills required
  • Must be comfortable working with multidisciplinary teams and physicians, navigating various insurance plans, and identifying community resources for patients
  • Must be a collaborative team player
  • At least 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN, required for consideration
  • Candidate DOB and last 4 digits of SSN required for consideration

Additional Information

  • Responsibilities include inpatient care coordination, utilization review, navigation of insurance plans, and connecting patients with appropriate resources and community programs
  • Supported by unit leadership, other case managers, a case management assistant, and hospitalists available 24/7
  • Orientation includes a new employee orientation (NEO) plus 3 shifts of department-specific orientation, with 1–2 additional shifts available if needed
  • Required dress code: eggplant-colored scrubs or business casual attire with a white lab coat
  • Free parking available on-site
  • First-time travelers are welcome to apply
  • Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months to be eligible
